Yearly Routine Physical Exams for Children and Adolescents
Children and Adolescents need a routine yearly physical exam to ensure that they are growing appropriately. These appointments are typically scheduled around the child’s birthday or just prior to the start of the school year.
The following services are provided during the routine yearly exam:
• Height and weight measurements
• Routine vaccinations
• Complete physical exam
• Developmental / school performance screening
• Nutrition assessment and dietary guidance
• Hearing and vision screening
• Sports, Day Care, and Camp physicals are offered when requested
• Screening blood work is ordered when appropriate*
Important Information Concerning Sports Physicals
If your child participates in sports, he or she should still have an annual physical exam. We feel strongly that a “sports physical” at school or urgent care center is not a replacement for an annual check-up.
If your child has not had a complete physical within the past 12 months, we will schedule your child for a complete physical and complete your sports forms during that visit. This physical will be filed to your insurance company and a co-pay/co-insurance will apply.
If your child has already had a checkup within the past 12 months: we will schedule a “sports physical”. Since insurance will NOT pay for two physicals per year, you will be required to pay $40.00 cash at the time of service. This visit will not be filed with your insurance company.
